Private Only Extended Fields

- Migration to add db column private_only:boolean to Topic type and content type field mappings
- Upgrade task to set all private_only values to false
- Add a "private only?" checkbox on topic/content type edit pages (with exception of user content type)
- Add JS to unselect and hide required checkbox if private checkbox ticked (plus ruby to change if needed)
- Edit the extended_field editor method to return '' if private_only? and editing public version
