Optimize Keyboard Navigation for Forms by adding tab indexes

Improve accessibility by adding tab indexes to certain forms in Kete, and to the navigation around the form (header and footer).

    The enhancement_access_keys_and_tab_indexes branch is looking good. i made a few adjustments, but almost ready to be merged to master.

    The only remaining issue that I can see is the site admin only form element for moving an item from one basket to another. Works for with tabindex for topics, but all other edit forms skip it. I took a whack by adding parens around :tabindex => '1', but didn't have any luck.

    See if you can find a solution, test edit forms again, and then go ahead and merge to master.

    Thanks Walter. Looks like tabindex was being passed in as a rails helper option, instead of an html options. The above work has been finished and merged into master. Resolving ticket.
